Most viewed - Stargardt's Macular Dystrophy - Severe Geographic Retinal Atrophy 69-year-old man has had vision loss in both eyes starting about ten years prior to when I saw him first in 2005. He was diagnosed at one point with Stargardt’s macular dystrophy. When I saw him, he had retinal atrophy in each eye. His vision unfortunately is still limited in both eyes. He was in the Viet Nam war and is concerned whether something might be linked to the service there, especially exposure to Agent Orange, which is an herbicide sometimes containing Dioxin.
VISUAL ACUITY: Vision is 20/200 in each eye. IOP: OD 16, OS 12. There is 3+ nuclear sclerosis in both eyes.
EXTENDED OPHTHALMOSCOPY:
OD: Vertical C/D ratio is 0.2. There is a well-circumscribed atrophic macular scar.
OS: Vertical C/D ratio is 0.2. There is a well-circumscribed atrophic macular scar.
OCT SCAN: The OCT scan shows retinal atrophy, even outside the scar in both eyes.
PHOTOGRAPHS: Photos confirm clinical findings.
FLUORESCEIN ANGIOGRAPHY: FA shows well-circumscribed areas of hyperfluorescence in the macula of each eye.
IMPRESSION: 1. MACULAR SCARS WITH ATROPHY – BOTH EYES
2. POSSIBLE STARGARDT’S MACULAR DYSTROPHY
3. POSSIBLE CHLOROQUINE
DISCUSSION: I explained to the patient the Agent Orange and related chemicals, as far as my understanding, do not have any effect on the macula and there are no reports of that. His retinas do look like what you would see in someone with Chloroquine toxicity and it is possible with his service that if he was on high-dose Chloroquine for any amount of time, that could have caused the retinal damage. I told him he could look into that. In the meantime, I asked him to return here for check in eighteen months.
