61-year-old man was seen in the office on 4/26/2011. He has noticed increased floaters in the right eye for about the last week. Since they began, they are getting a little less bothersome. When they came on, he was not doing anything strenuous and he was not taking any blood thinners. He does take multivitamins. VISUAL ACUITY: Vision OD is 20/160, OS 20/30. IOP: OD 15, OS 17. SLIT LAMP EXAM: There is 1+ nuclear sclerosis in both eyes. EXTENDED OPHTHALMOSCOPY: OD: Vertical C/D ratio is 0.5. There is a posterior vitreous separation. There is 3+ vitreous hemorrhage with a retinal tear, about 2-disc diameters posterior to the equator at 10:00 o’clock over a retinal vessel. OS: Vertical C/D ratio is 0.5. There is no posterior vitreous separation. The macula and periphery look healthy. IMPRESSION: 1. VITREOUS HEMORRHAGE – RIGHT EYE 2. RETINAL TEAR – RIGHT EYE 3. POSTERIOR VITREOUS SEPARATION – RIGHT EYE 4. CATARACTS DISCUSSION: I explained to the patient he does have a retinal tear in the right eye with fresh vitreous hemorrhage. I treated the retinal tear with laser. I asked him to avoid heavy lifting and straining to prevent a recurrent vitreous hemorrhage, and I asked him to avoid vibration or jarring activity to prevent retinal detachment. I asked him to return for check in a week, sooner should he notice any striking change.
