Most viewed - Diabetic Macular Edema - Exudates 56-year-old woman was seen on 3/3/08. She has diabetes for twelve years. She has noticed blurring of the vision in the right eye for about six months. Her left eye is a little bit blurred as well. You saw a problem with the retina and suggested she come in here for evaluation.
VISUAL ACUITY: Vision OD is 20/70, OS is 20/25. IOP: 14 OD, 16 OS.
SLIT LAMP EXAM: There is 1+ nuclear sclerosis in both eyes.
EXTENDED OPHTHALMOSCOPY:
OD: Vertical C/D ratio is 0.3. There is a posterior vitreous separation. There is clinically significant macular edema involving the fovea. There are 2+ retinal hemorrhages and microaneurysms with no proliferation.
OS: Vertical C/D ratio is 0.3. There is a posterior vitreous separation and clinically significant macular edema touching the fovea temporally. There are 2+ retinal hemorrhages and microaneurysms with no proliferation.
OCT SCAN: There is clinically significant macular edema in both eyes.
FLUORESCEIN ANGIOGRAPHY: Photos confirm clinical findings. FA shows multiple leaking microaneurysms in both eyes.
IMPRESSION: 1. BACKGROUND DIABETIC RETINOPATHY – BOTH EYES
2. CLINICALLY SIGNIFICANT MACULAR EDEMA – BOTH EYES
DISCUSSION: I explained to the patient with focal laser we can reduce the risk of vision loss in both eyes and possibly improve the vision. I treated her right eye without any difficulty. She will return for check in one month for probable treatment of the other eye. I told her if the laser does not sufficiently dry up the macula, there are other treatments we could add to it.
