Last additions - Diabetic Macular Edema Fresh Laser 2 60-year old man has diabetic retinopathy in both eyes. His vision in the right eye is getting a little bit worse. He most recently had laser treatment in October of 2007 in both eyes.
VISUAL ACUITY: OD 20/60, OS 20/50. IOP: OD 16, OS 15. There is 2+ nuclear sclerosis in both eyes.
EXTENDED OPHTHALMOSCOPY:
OD: Vertical C/D ratio is 0.1. There are patchy microaneurysms and laser to retinal hemorrhages. There are exudates and cotton wool spots.
OS: Vertical C/D ratio is 0.1. Similarly, there are microaneurysms and hemorrhage and laser.
OCT SCAN: The OCT scan shows central foveal thickness in the right eye of 338 microns, with significant parafoveal thickening. The left eye has parafoveal thickening with a normal foveal thickness.
Photos confirm clinical findings.
FLUORESCEIN ANGIOGRAM: FA shows multiple microaneurysms in both eyes.
IMPRESSION:
1. BACKGROUND DIABETIC RETINOPATHY – LEFT EYE
2. MACULAR EDEMA, WORSE IN THE RIGHT EYE THAN THE LEFT EYE
DISCUSSION: I explained to the patient that the macula in the right eye is significantly more swollen than it has been in the past. I treated the right eye with focal laser. I asked him to return for a check in three or four months. I will re-evaluate both eyes and I may add a little laser to the left eye at that time.
