Last additions - Post-Retinal Detachment - Proliferative Vitreoretinopathy (PVR) - Panretinal Retinoschisis 46-year-old man had a complex retinal detachment repair in the left eye by me initially December 9th of 2011. Then I removed his oil January 4th of 2012 and he redetached requiring surgery February 4th of 2012. I just removed his oil October 31st of 2012. He has had Kenalog injections because of increasing retinal edema. His vision is still hazy in the left eye at a distance.
VISUAL ACUITY: OS 20/125. Pinhole 20/63. IOP: OD 14, OS 17. The left eye has a posterior chamber intraocular lens in good position.
EXTENDED OPHTHALMOSCOPY:
OS: Vertical C/D ratio is 0.2. The macula looks edematous throughout.
SPECTRALIS-SD-OCT SCAN: The OCT scan does show retinoschisis in the macula and throughout the periphery. There is no retinal detachment though.
IMPRESSION:
1. PROBABLE TRACTIONAL PAN RETINAL SCHISIS – LEFT EYE
DISCUSSION: I explained to the patient with intravitreal Kenalog there is a possibility of quieting the eye down and possibly improving the vision. The office visit included a 15-minute counseling session during which I reviewed the alternatives, risks, benefits, and limitations of various treatment options. I injected the eye with Kenalog (0.1 cc) without any difficulty.
