Last additions - Juxtafoveal Telangiectasis - Stage 5 Spectralis 60-year-old man has juxtafoveal retinal telangiectasis in both eyes with vision change. Recently his vision has been declining. He had more trouble reading. I last saw him in March of 2010. Since then he notices he is not as good as he was.
VISUAL ACUITY: OD 20/50, OS 20/70. IOP: OD 11, OS 12. The posterior chamber intraocular lens is in good position in both eyes.
EXTENDED OPHTHALMOSCOPY:
OD: Vertical C/D ratio is 0.3. There is no posterior vitreous separation. There are patchy telangiectatic vessels and retinal crystals.
OS: Vertical C/D ratio is 0.5. There is no posterior vitreous separation. There is hemorrhage in the macula with pigment epithelium thickening superonasal to the fovea.
SPECTRALIS-SD-OCT SCAN: The OCT scan of the right eye shows a normal central foveal thickness of 289 microns and the left eye shows foveal edema with juxtafoveal edema. The average central foveal thickness was 330 microns. (NOTE: Retinal thickness measured with Spectralis OCT is approximately 70 μm greater than that measured with Stratus OCT. This increased measurement corresponds to the inclusion of the outer segment-RPE-Bruch's membrane complex by Spectralis OCT.) Photos confirm clinical findings.
FLUORESCEIN ANGIOGRAPHY: Fluorescein angiography shows bright hyperfluorescence more in the macula in the left eye than the right eye.
IMPRESSION:
1. JUXTAFOVEAL RETINAL TELANGIECTASIS – BOTH EYES
2. STAGE FIVE – LEFT EYE
3. WET AGE-RELATED MACULAR DEGENERATION – LEFT EYE
DISCUSSION: I explained to the patient with Avastin there is a chance of drying the macula in the left eye and improving the vision. I treated the left eye with intravitreal injection of Avastin (1.25 mg/0.05 ml) without any difficulty today.
