Juxtafoveal Telangiectiasis - Stage 3 - Gray Ring - MacTel

59-year-old woman was seen in the office on January 26, 2012. She has noticed decreasing vision in the right eye for a few years. She had cataract surgery done and although the vision in the left eye is excellent, the vision in the right eye has been a little bit disappointing. She has had diabetes for five years. She has eight siblings, as far as she knows everybody’s vision is good, accept for the occasional need for glasses.
VISUAL ACUITY: OD 20/100, Pinhole 20/40. OS 20/20. IOP: OD 14, OS 10.
SLIT EXAMINATION: The posterior chamber intraocular lens is in good position in both eyes.
EXTENDED OPHTHALMOSCOPY:
OD: Vertical C/D ratio is 0.1. There is no posterior vitreous separation. There is a grey tint to the retina. There are telangiectatic vessels temporal to the fovea and retinal crystals just on the temporal edge of the fovea.
OS: Vertical C/D ratio is 0.1. There is no posterior vitreous separation. The fovea looks normal. There is a gray tint to the macula.
FUNDUS AUTO FLUORESCENCE: The fundus auto fluorescence images show some loss of macular pigment in the temporal region of both eyes with telangiectasis to the vessels.
SPECTRALIS-SD-OCT SCAN: The OCT scan shows gaps and cysts in the retina in both eyes with a normal central foveal thickness and photos confirm clinical findings.
IMPRESSION:
1. IDIOPATHIC JUXTAFOVEAL RETINAL TELANGIECTASIS GROUP TWO A – BOTH EYES, STAGE THREE RIGHT EYE AND STAGE TWO IN THE LEFT EYE
DISCUSSION: I explained to the patient she does have juxtafoveal retinal telangiectasis in both eyes. At the moment there is no treatment that actually benefits this disease, but most people with it don’t lose substantial vision, at least in one eye.
