Juxtafoveal Telangiectasis - Stage 3 AND Juxtapapillary Asymptomatic Choroidal Neovascular Membrane (Wet AMD)

84-year-old woman has juxtafoveal retinal telangiectasis in both eyes. The right eye has an asymptomatic extra foveal juxtapapillary choroidal neovascular membrane. Her vision since she was here last is about stable. The left eye does have a cataract.
VISUAL ACUITY: OD 20/400, OS 20/63. IOP: OD 12, OS 11. The right eye has a posterior chamber intraocular lens in good position. The left eye has 3+ nuclear sclerosis.
EXTENDED OPHTHALMOSCOPY:
OD: Vertical C/D ratio is 0.2. There is a juxtapapillary choroidal neovascular membrane. There are crystals and pigment migration in the macula.
OS: There are crystals and pigment in the macula.
SPECTRALIS-SD-OCT SCAN: The OCT scan in the right eye shows the juxtapapillary choroidal neovascular membrane and the foveal defects corresponding to the juxtafoveal telangiectasis. The juxtafoveal membrane in the right eye is causing some irregular hyperfluorescence around it, but the entire lesion stops well short of the center of the fovea, probably at least a millimeter away. In the left eye the macula has some defects, but there is no evidence of any intraretinal or subretinal fluid.
IMPRESSION:
1. JUXTAFOVEAL TELANGIECTASIS – BOTH EYES
2. JUXTAPAPILLARY EXTRA FOVEAL ASYMPTOMATIC
CHOROIDAL NEOVASCULAR MEMBRANE – RIGHT EYE
DISCUSSION: I explained to the patient the maculae both look stable. I asked her to return here for a check in a year or sooner should she notice a problem and see you periodically. Thank you for allowing me to participate in her care.
