70-year-old man was seen in the office on 6/29/2010. He has had blurred vision in the right eye for about one or two months. You saw a problem with macular edema and branch retinal vein occlusion and suggested he come here for an evaluation. He has high blood pressure for which he takes medications. VISUAL ACUITY: Vision OD is 20/80, OS is 20/25. IOP: OD 11, OS 19. AMSLER GRID: Normal in both eyes. SLIT LAMP EXAM: There is 2+ nuclear sclerosis in both eyes. EXTENDED OPHTHALMOSCOPY: OD: Vertical C/D ratio is 0.2. There is no posterior vitreous separation. There is a superotemporal macular branch retinal vein occlusion with 2+ macular edema. OS: Vertical C/D ratio is 0.2. There is a posterior vitreous separation, 1+ macular drusen, and 2+ equatorial drusen. There are also 2+ arteriovenous crossing changes. OCT SCAN: The OCT scan shows macular edema in the right eye. The left eye has a normal central foveal thickness. The average central foveal thickness in the right eye is 418 microns. Also, there is a serous macular detachment in the right eye. PHOTOGRAPHS: Photos confirm clinical findings. FLUORESCEIN ANGIOGRAPHY: FA shows superotemporal macular branch retinal vein occlusion in the right eye. There are some areas of nonperfusion near the fovea and there is leakage in the fovea in the late frames. The right eye has a few staining drusen. IMPRESSION: 1. MILD MACULAR DEGENERATION – LEFT EYE 2. MACULAR BRANCH RETINAL VEIN OCCLUSION – RIGHT EYE 3. MACULAR EDEMA – RIGHT EYE 4. CATARACTS DISCUSSION: I explained to the patient there is a branch retinal vein occlusion in the right eye. There is about a 50% chance this will start to improve on its own. If it does not, then treatment would be reasonable to consider, either with laser, injections, or both.
