55-year-old woman has myopic macular degeneration in both eyes. She has recently noticed a problem with her vision and an Amsler grid change starting a few days ago in the left eye. VISUAL ACUITY: OD: 20/60. PH: 20/25. OS: 20/200. PH: 20/30. IOP: OS: 13. OS: There is 1+ nuclear sclerosis. EXTENDED OPHTHALMOSCOPY: OS: Vertical C/D ratio is 0.1. There is a posterior staphyloma. There are pigment irregularities in the macula. There is no subretinal fluid, hemorrhage or exudate. The OCT scan was unreliable. A fluorescein angiogram shows no leakage from any of the macular lesions. IMPRESSION: 1. MYOPIC MACULAR DEGENERATION IN THE LEFT EYE. 2. AMSLER GRID CHANGES IN THE LEFT EYE. DISCUSSION: I explained to the patient that the macula looks okay, but since she has noticed a change I asked her to return again in six to eight weeks.
