83-year-old woman was seen in the office on September 19, 2011. She noticed some decreased vision in the right eye for about a month. She is not doing badly though. It is not bothering her visual function. She is able to get around, read and see fine. You saw a problem with the macula in the right eye and suggest she come here for an evaluation. She did was told she had a macular pucker in the eye, somewhere around ten or twenty years ago. You noticed recently though a change there and suggest she come here for an evaluation. VISUAL ACUITY: OD 20/50, OS 20/20. IOP: OD 12, OS 13. SLIT EXAMINATION: The posterior chamber intraocular lens is in good position in both eyes with open capsules. EXTENDED OPHTHALMOSCOPY: OD: Vertical C/D ratio is 0.1. There is posterior vitreous separation. There is 2+ epiretinal membrane. There is a rolled edge of an epiretinal membrane on the inferior edge of the fovea. There is also a 2 millimeter benign choroidal nevus in the inferotemporal periphery with drusen over it. OS: Vertical C/D ratio is 0.1. There is posterior vitreous separation and 1+ macular drusen. Photos confirm clinical findings. SPECTRALIS-SD-OCT SCAN: The OCT scan of the right eye does show the macular pucker. It is not a full thickness macular hole, but there is a pseudo macular hole and the left eye shows a few macular drusen. Photos confirm clinical findings of the nevus in the right eye. IMPRESSION: 1. MACULAR PUCKER – RIGHT EYE 2. CHOROIDAL NEVUS – RIGHT EYE 3. MACULAR DRUSEN – LEFT EYE DISCUSSION: I explained to the patient the macular pucker in the right eye is starting to peel some and that is part of the reason why her vision has changed. Her vision function is reasonable to the point where I don’t think surgery at the moment is necessary or indicated, but this might change. I suggest she return for a check in six to eight weeks or sooner should she notice a problem and see you periodically. FU: VA improved to 20/30 at 2 months
