89-year-old man has stage IV juxtafoveal retinal telangiectasis in both eyes with background diabetic retinopathy. His vision is stable since he was here last which is about 6 months ago.
VISUAL ACUITY: OD 20/50, OS 20/70. IOP: OD 14, OS 11. The posterior chamber intraocular lens is in good position in both eyes with open capsules.
EXTENDED OPHTHALMOSCOPY:
OD: Vertical C/D ratio is 0.0. There is a black spot temporal to the fovea.
OS: Vertical C/D ratio is 0.0. There is a black spot temporal to the fovea. There are rare retinal hemorrhages in both eyes.
OCT SCAN: The OCT scan confirms an absence of intraretinal or subretinal fluid.
IMPRESSION:
1. STAGE IV JUXTAFOVEAL RETINAL TELANGIECTASIS – BOTH EYES
2. BACKGROUND DIABETIC RETINOPATHY - BOTH EYES
DISCUSSION: I explained to the patient the maculae are both stable. I asked him to return for check in a year, sooner should he notice a problem.
