Drusen - Basal Laminar and Equatorial

70 year old woman has had macular drusen since her thirties and she said her sister has a similar problem. Recently the vision in the left eye has substantially declined. I saw her back in April of 2008.
VISUAL ACUITY: Her vision is OD 20/16, OS 10/200. IOP: OD 17, OS 19. The posterior chamber intraocular lens is in good position in both eyes. The left eye has an open capsule.
EXTENDED OPHTHALMOSCOPY:
OD: Vertical C/D ratio is 0.1. There are 2+ macular drusen. There is no subretinal fluid, hemorrhage, or exudate.
OS: Vertical C/D ratio is 0.1. There are 2+ macular drusen. There is an area of geographic atrophy, 2-disc areas in size, centered temporal to the fovea which involves the foveal center.
OCT SCAN: There is no intraretinal or subretinal fluid in either eye. There is, however, retinal atrophy, more in the left eye than the right.
PHOTOGRAPHS: Photos confirm clinical findings.
FLUORESCEIN ANGIOGRAPHY: FA shows speckled hyperfluorescence consistent with basal laminar drusen with a radiating pattern out in the far periphery and hyperfluorescence in the macula of the left eye, consistent with geographic atrophy. There is no evidence of any neovascularization in either eye.
IMPRESSION: 1. BASAL LAMINAR DRUSEN – BOTH EYES
2. FOVEAL GEOGRAPHIC ATROPHY – LEFT EYE
3. DRY MACULAR DEGENERATION – BOTH EYES
DISCUSSION: I explained to the patient the left eye does unfortunately have increasing retinal atrophy. At the moment there is no treatment for that, but in time there may be something we can offer.
