Scar - Radiating Chorioretinal Folds Disciform Scar

79-year-old woman has wet age-related macular degeneration in both eyes. The left eye had intravitreal injections of Avastin starting in May of 2009. Her vision remains good in the left eye and poor in the right eye.
VISUAL ACUITY: OD 8/200, OS 20/60. IOP: OD 10, OS 11. The posterior chamber intraocular lens is in good position in both eyes.
EXTENDED OPHTHALMOSCOPY:
OD: Vertical C/D ratio is 0.5. There is a macular scar centrally.
OS: Vertical C/D ratio is 0.3. There are patchy hyperpigment spots in the macula. Photos confirm clinical findings.
FLUORESCEIN ANGIOGRAPHY: Fluorescein angiography of the right eye shows radiating chorioretinal folds around the macular scar. In the left eye there are hyperpigment spots around the macula with hyper and hypofluorescence. There is no evidence of any neovascularization.
IMPRESSION:
1. WET AGE-RELATED MACULAR DEGENERATION – LEFT EYE
2. INVOLUTED SUBFOVEAL CHOROIDAL NEOVASCULAR MEMBRANE – LEFT EYE
3. MACULAR SCAR – RIGHT EYE
DISCUSSION: I explained to the patient the macula in the left eye remains dry. I treated the left eye with intravitreal injection of Avastin (1.25 mg/0.05 ml) without any difficulty today. I asked her to return in two months or sooner if she should notice a problem and see you back periodically.
