Diabetic Macular Edema Fresh Laser

74-year-old man has diabetic macular edema in both eyes and background diabetic retinopathy. His vision has blurred over the last few months. His blood sugars have been fluctuating.
VISUAL ACUITY: Vision OD is 20/25, OS is 20/80. IOP: OD 10, OS 9. The right eye has a posterior chamber intraocular lens in good position. The left eye has a corneal scar a long the superior edge of the cornea. The posterior chamber intraocular lens is in good position.
EXTENDED OPHTHALMOSCOPY:
OD: Vertical C/D ratio is 0.6. There is edema temporal to the fovea with microaneurysms.
OS: Vertical C/D ratio is 0.6. There is edema temporal to the fovea with microaneurysms.
OCT SCAN: The OCT scan in the right eye shows normal macular thickness. The left eye has edema, particularly temporal to the fovea. Photos confirm clinical findings.
FLUORESCEIN ANGIOGRAM: FA shows multiple microaneurysms predominately temporal to the fovea with late leakage into the fovea in the left eye. The right eye has microaneurysms but no significant leakage into the fovea.
IMPRESSION:
1. BACKGROUND DIABETIC RETINOPATHY – BOTH EYES
2. CLINICALLY SIGNIFICANT MACULAR EDEMA – LEFT EYE
DISCUSSION: I explained to the patient with focal laser we can reduce the risk of vision loss in the left eye. I treated that eye today without any difficulties.
