59-year-old woman was seen in the office on 1/4/2011. She has had blurry vision in the right eye for about the last one or two years. She notices it mostly in the morning when she is reading the newspaper. Later in the day it is not as bad. Also, she notices after she washes her face at night, she cannot read then either. She has a dilated pupil she has had since her cataract surgery done in 1981. She had cataract surgery in 1981, a retinal detachment repair with scleral buckle in 1988 in the right eye, and then she had lens implants in both eyes, anterior chamber lenses, in 1994. She is on Restasis drops in both eyes. She did start taking Latisse for eyelashes about a year and a half ago. VISUAL ACUITY: Vision OD is 20/40, OS is 20/20. IOP: OD 20, OS 19. The right eye has an anterior chamber intraocular lens, which is tucked, and also a fixed dilated pupil at about 8 mm. The left eye has an anterior chamber intraocular lens nicely positioned. EXTENDED OPHTHALMOSCOPY: OD: Vertical C/D ratio is 0.5. The macula looks slightly irregular. There is also a 2+ epiretinal membrane. OS: Vertical C/D ratio is 0.5. The macula is nice and smooth. OCT SCAN: The OCT scan shows slight retinal thickening, worse in the right eye than the left eye. The average central foveal thickness in the right eye is 380 microns and the left eye 247 microns. PHOTOGRAPHS: Photos confirm clinical findings. FLUORESCEIN ANGIOGRAPHY: FA of the right eye shows minimal leakage at the fovea and optic nerve in the late frames. IMPRESSION: 1. CYSTOID MACULAR EDEMA – RIGHT EYE 2. MACULAR PUCKER – RIGHT EYE 3. ANTERIOR CHAMBER INTRAOCULAR LENS – BOTH EYES 4. DILATED PUPIL WITH TUCKED HAPTICS OF THE INTRAOCULAR LENSES -- RIGHT EYE DISCUSSION: I explained to the patient that the macular edema in the right eye could very well be from her intraocular lenses, but it is unusual to happen this late after her most recent procedure. It is possible it may be from the Latisse. I asked her to stop taking Latisse and then return in two months to see how she looks. 4 months later: 5/24/2011. This pleasant 59-year-old woman has a macular pucker in the right eye. She has had in that eye retinal detachment repair in 1988 and she has had cataract lens implants done in 1994. Since the vision blurred, I was a little concerned about her Latisse, which is related to the prostaglandin drops, and I thought might cause macular edema. She stopped it for awhile and she is back on it again. She recently updated her prescription and now sees rather well. VISUAL ACUITY: Vision OD is 20/25
