CNVM - Juxtapapillary Extrafoveal CNVM Serous Macular Detachment

87-year-old woman has wet age-related macular degeneration in the left eye with an extrafoveal choroidal neovascular membrane. A retina specialist treated her with Lucentis in March, April, and May of last year. Her vision has been doing fine and her left eye is seeing okay.
VISUAL ACUITY: Vision OD is 20/25, OS is 20/25. IOP: OD 16, OS 18. The left eye has a posterior chamber intraocular lens is in good position.
EXTENDED OPHTHALMOSCOPY:
OS: Vertical C/D ratio is 0.3. The exudative choroidal neovascular membrane nasal to the fovea looks substantially worse.
OCT SCAN: The OCT scan shows subretinal fluid just touching the fovea, which has substantially declined compared to last visit.
FLUORESCEIN ANGIOGRAPHY: There is a juxtapapillary choroidal neovascular membrane nasal to the optic nerve, which is wrapping around the superior portion of the nerve and leaking heavily in the late frames.
IMPRESSION: 1. WET AGE-RELATED MACULAR DEGENERATION – LEFT EYE
2. SEROUS MACULAR DETACHMENT FROM AN EXTRAFOVEAL CHOROIDAL NEOVASCULAR MEMBRANE – LEFT EYE
DISCUSSION: I explained to the patient, given the distance of the choroidal neovascular membrane from the fovea and the fact that the subretinal fluid from it is now affecting the fovea, I suggest we treat her with choroidal laser. I treated the area nasal to the optic nerve with moderately heavy choroidal laser. I will have her return for check in one month. Hopefully that will be enough to dry out the macula. If not, I can always add to it in the future.
