Chronic Central Serous Retinopathy treated with Photodynamic Laser - Visudyne

48-year-old man has had waxing and waning vision in the left eye for the last year from central serous retinopathy. I initially saw him for this October 4th. He said the right eye had an episode about fifteen years ago, which lasted a few weeks, and his left eye has been poor since November of 2006. He was being followed elsewhere for this and is on Diamox in his treatment. I stopped the Diamox. His vision in the left eye is fluctuating and possibly a little worse since he was here last.
VISUAL ACUITY: Vision OD is 20/16, OS is 20/30. IOP: OD 20, OS 20. The lenses are clear.
EXTENDED OPHTHALMOSCOPY:
OD: Vertical C/D ratio is 0.3. The macula is dry.
OS: Vertical C/D ratio is 0.3. There is a serous macular detachment.
OCT SCAN: The OCT scan of the left eye shows increased subretinal fluid.
Photos confirm clinical findings.
FLUORESCEIN ANGIOGRAPHY: The right eye shows a few patches of hyperfluorescence superotemporal to the optic nerve. In the left eye, there is a leaky spot just inferotemporal to the fovea, which increases substantially in the late frames. There is also a leaky spot under the fovea.
IMPRESSION:
1. CENTRAL SEROUS RETINOPATHY – BOTH EYES
2. ACTIVE CENTRAL SEROUS RETINOPATHY – LEFT EYE
3. SEROUS MACULAR DETACHMENT – LEFT EYE
DISCUSSION: I explained to the patient given the location and configuration of the abnormal blood vessels associated with the central serous retinopathy, the best treatment at this moment for him is photodynamic laser.
2 months after Photodynamic laser to the left eye the vision was up to 20/16 and stayed good for 2 years so far.
