Acute Central Serous Retinopathy

48-year-old man who I saw in the office on February 12, 2009. He has had hazy vision in the right eye for about three days. He has a little more of a headache than usual. He is under a fair amount of stress. He does not take any steroids that he knows of. He does use a nasal spray, which he is not sure if that is a steroid spray or not. He is going to call us with that medicine. I checked his blood pressure today is was 115/95.
VISUAL ACUITY: OD 20/15, OS 20/25. IOP: OD 14, OS 15.
SLIT EXAMINATION: Normal with a clear lens.
EXTENDED OPHTHALMOSCOPY:
OD: Vertical C/D ratio is 0.1. There is no posterior vitreous separation. There are two discreet pigment epithelial detachments superior to the fovea. There is no intraretinal or subretinal fluid.
OS: Vertical C/D ratio is 0.1. There is a pigment epithelial detachment superior to the fovea. There is 2+ subretinal fluid.
OCT SCAN: The OCT scan confirms the right macula to be dry. The left eye has subretinal fluid. Photos confirm clinical findings.
FLUORESCEIN ANGIOGRAM: The fluorescein angiogram shows leakage from a pigment epithelial detachment inferior to the fovea in the left eye filling the serous macular detachment. The right eye has staining in the pigment epithelial detachment superior to the fovea.
IMPRESSION:
1. IDIOPATHIC CENTRAL SEROUS RETINOPATHY – BOTH EYES
2. SEROUS MACULAR DETACHMENT – LEFT EYE
DISCUSSION: I explained to the patient that he does have central serous retinopathy in the left eye, ninety to ninety five percent of the time. Patients such as he will improve without needing any treatment. I asked him to return here for a check in six weeks or sooner should he notice a problem and see you regularly. Thank you for allowing me to participate in his care.
