65 year old female Renal disease with hypertension since age 18.
Medical Hx: Kidney Dialysis. Kidney Disease. Osteoporosis.
Surgical Hx: Tubal Ligation. 14 arm sx. aneurism left arm sx. back injections.
Systemic Meds: Prograf 1mg bid. Cellcept 250mg bid. Losartan 25mg qday. Famotideine 20mg bid. Fosomax, 70mgs qweek. Acyclovir 200mg bid. Vitamin D2. Bupropion HCL 300mgs. Clonazepam 1mg. Temazepam 15mg. Bumetanide 1mg. Estradid.
VA OD: Dcc20/40-2 PHNI NccJ5
VA OS: Dcc20/63 PHNI NccJ7
IOP: TP: OD:9 OS:10
