Pseudophakic CME - Recurrent twice off topical therapy

74 year old man - The left eye had had blurred vision for one year. He has been on PF QID for about a month then after a week the vision cleared and he decreased his drops to once daily. He was on another drop which he doesn't remember. (may have been prolensa daily) Then when the patient went to see you for the follow-up the patient noticed the vision was blurred. His cataract surgery was 14 month ago. He is back up to QID drops in the left eye for about a week.
Medical Hx: Heart Disease. Hypertension, Systemic. Cartoid Artery Disease. Atrial Fibrillation. neuropathy (Knees down). Enlarged Prostate.
Surgical Hx: Knee Surgery (Left 2014). Heart Stent (2).
Systemic Meds: LOSARTAN. potassium. alfuzosin hydrochloride 10 MG 24 HR Extended Release Oral Tablet. atorvastatin (as atorvastatin calcium) 40 MG Oral Tablet. omeprazole 20 MG Delayed Release Capsule. COQ10. GLUCOSAMINE/CHONDROITIN. CENTRUM SILVER. MONTELUKAST. ZYRTEC. Zydenafil. DILTIAZEM. ISOSORBIDE MONONITRATE. Eliquis 5 MG Oral Tablet. GABAPENTIN. Testosterone Cypionate, cypionate 200 mg/mL intramuscular solution. CLOPIDOGREL. FINASTERIDE.
VA OD: sc20/20-1 NccJ1-1
VA OS: sc20/63 PH20/50-2 NccJ7
IOP: TP: OD:16 OS:18
The patient’s vision substantially declines with the CME. The patient treated initially with drops (PF and Ketorolac QID) and did OK but then had trouble with compliance. Had PST Kenalog but IOP went to 30. Then failed taper to QD PF Ketorolac twice. Now on BID PF, Ketorolac, and Dorz/Timolol (for steroid response) . Moved out of town.
