66-year-old man has had laser to retinal tears in both eyes in 1999 and in 2001. His right eye has had rapidly declining vision over the last month. VISUAL ACUITY: OD: 20/200; OS: 20/50. IOP: OD: 13, OS: 11. The posterior chamber intraocular lens is in good position in both eyes. The right eye has an open capsule. EXTENDED OPHTHALMOSCOPY: OD: Vertical C/D ratio is 0.3. There is 2+ epiretinal membrane. There is laser nasally. OS: Vertical C/D ratio is 0.3. The macula is flat and dry. There is laser superonasally. OCT scan of the right eye shows increased macular thickening from the macular pucker with a central foveal thickness of 401 microns, which is substantially worse than 285 microns in October. The left eye has a normal foveal contour. A fluorescein angiogram of the right eye shows minimal leakage from the retinal vessels from the macular pucker. The angiogram in the left eye is normal. Photos confirm clinical findings. IMPRESSION: 1. MACULAR PUCKER - RIGHT EYE 2. WORSENING TRACTIONAL THICKNESS OF THE FOVEA IN THE RIGHT EYE WITH VISION LOSS 1. HISTORY OF LASER TO RETINAL TEARS AND DETACHMENTS - BOTH EYES. DISCUSSION: I explained to the patient that the macular pucker in the right eye is adversely effecting his vision. I suggested he consider vitrectomy for that. He is going to return here in January. Unless there is a substantial improvement, I will operate on that probably in January. I will have him see you back regularly as well. Follow up after surgery vision is improved from 20/200 to 20/30: February 22, 2008. This pleasant 66-year-old man had vitrectomy in the right eye for a macular pucker January 9th. His vision is improving nicely. VISUAL ACUITY: OD 20/30. IOP: OD 11. I asked him to return for a check in two months, sooner should he notice any problem.
