Diabetic Macular Edema and Fresh Laser Left Eye

82-year-old woman in the office on March 14, 2008. She has been diabetic for some time. Her last eye exam was about five years ago up in the Detroit area. She has moved to Florida since you just saw her for the first time recently and noticed a problem with her retina and suggested she come in here for evaluation. She was recently hospitalized and just released for anemia. Her hemoglobin was terrible low. She has been started on some new medication for that.
VISUAL ACUITY: OD 20/60, OS 20/70. IOP: OD 16, OS 17.
SLIT LAMP EXAM: The posterior chamber intraocular lens is in good position in both eyes.
EXTENDED OPHTHALMOSCOPY:
OD: Vertical C/D ratio is 0.3. There is posterior vitreous separation. There are 2+ retinal hemorrhages.
OS: Vertical C/D ratio is 0.3. There is posterior vitreous separation. There are 2+ retinal hemorrhages. There is edema temporal to the fovea.
OCT SCAN: The OCT scan of the right eye shows retinal atrophy with no intraretinal or subretinal fluid. The left eye has edema temporal to the fovea with involvement of the fovea. Photos confirm clinical findings.
FLUORESCEIN ANGIOGRAM: FA shows multiple microaneurysms in both eyes.
IMPRESSION:
1. BACKGROUND DIABETIC RETINOPATHY – BOTH EYES
2. CLINICALLY SIGNIFICANT MACULAR EDEMA – LEFT EYE

DISCUSSION: I explained to the patient with focal laser we can reduce the risk of vision loss in the left eye and possibly even improve the vision. I treated the left eye with focal laser today without any difficulty. She will return here for a check in three months, sooner should she notice any problem.
