58-year-old man was seen in the office on March 8, 2010. He has noticed his vision in the right eye not quite normal and then about a year ago he had cataract surgery done and notices since then the vision is still not normal. The vision started to him at least five years ago. He does have a history of tinnitus, because he has had problems with his ears. He notices for now he has problems with sort of light and dark adaptations, where he sees a bright light and looks away from the bright light, his vision is washed out for over a minute. He is not diabetic. VISUAL ACUITY: OD 20/40, OS 20/40. IOP: OD 15, OS 12. SLIT EXAMINATION: The posterior chamber intraocular lens is in good position in both eyes. The right eye has an open capsule. EXTENDED OPHTHALMOSCOPY: OD: Vertical C/D ratio is 0.5. There is no posterior vitreous separation. There is settled retinal crystals. There are telangiectatic vessels on the temporal edge of the fovea. OS: Vertical C/D ratio is 0.5. There is no posterior vitreous separation. There is settled retinal crystals. There are telangiectatic vessels on the temporal edge of the fovea. OCT SCAN: The OCT scan shows typical foveal gaps associated with a normal foveal contour and thickness. Photos confirm clinical findings. FLUORESCEIN ANGIOGRAPHY: Fluorescein angiography shows leakage of the paramacular capillaries with telangiectasis. IMPRESSION: 1. GROUP 2 JUXTAFOVEAL RETINAL TELANGIECTASIS STAGE 3 – BOTH EYES DISCUSSION: I explained to the patient he does have retinal telangiectasis in both eyes. Fortunately most patients with this problem maintain reasonably good vision in at least one eye till the end of their life. I did warn him to return urgently should he notice a striking change in either eye and I asked him to see you back regularly. I suggest he return here again in about a year and a half.
